НЕВРИТ ЗРИТЕЛЬНОГО НЕРВА АССОЦИИРОВАННЫЙ С МАЛЬФОРМАЦИЕЙ АРНОЛЬДА-КИАРИ

Abstract

Неврит зрительного нерва вызывается воспалительным, инфекционным либо демиелинизирующим процессом, а также интоксикацией. Примерно в 30% случаев оптических невритов этиология остается неясной.Представлен клинический случай оптического неврита на фоне мальформации Арнольда – Киари (МАК). Аномалия Арнольда-Киари – врожденный порок развития краниовертебральной зоны, включающий различные варианты смещения миндалин мозжечка ниже уровня большого затылочного отверстия. Наиболее частые офтальмологические проявления МАК включают: диплопию, сходящееся паралитическое косоглазие, монолатеральный горизонтальный или вертикальный нистагм, сужение периферических границ поля зрения, клонический блефароспазм, застойные диски зрительных нервов. На сегодняшний день отсутствуют публикации о случаях ассоциации неврита зрительного нерва с МАК.Установлено, что у больной М. наиболее вероятной причиной развития оптического неврита является МАК. Для раннего выявления этиологии и патогенеза папиллита, большое значение имеет МРТ головного мозга.
