Aspirin Discontinuation at 24 to 28 Weeks’ Gestation in Pregnancies at High Risk of Preterm Preeclampsia: A Randomized Clinical Trial

Abstract

(JAMA. 2023;329:542–550) Globally, 70,000 maternal deaths and 500,000 fetal deaths each year can be attributed to preeclampsia, which affects roughly 2-4% of pregnancies. Diagnosis of preeclampsia falls into several categories depending on gestational age at diagnosis, with early-onset preeclampsia (delivery <34 wk), preterm preeclampsia (delivery <37 wk), and term preeclampsia (delivery at or past 37 wk). Many serious complications can be associated with a diagnosis of preeclampsia, including preterm birth, fetal growth restriction, placental abruption, seizures, and others, with earlier onset correlating with more severe complications. Previous research has identified aspirin as an effective way to reduce the incidence of preeclampsia in high-risk individuals, especially when treatment is initiated in the late first trimester or early second trimester.
