Vorhofohrverschluss links – eine Alternative zur Dauerantikoagulation bei Vorhofflimmern?

Abstract

Zusammenfassung. Etwa 25% aller ischämischen Insulte werden durch kardiale Embolien bei Vorhofflimmern verursacht. Bei Patientinnen und Patienten mit Vorhofflimmern wird deshalb vor einer Elektrokonversion die orale Antikoagulation eingeleitet oder eine transösophageale Echokardiografie durchgeführt, um Blutgerinnsel im Herzen auszuschliessen. Dabei zeigt sich, dass praktisch alle kardialen Thromben (>90%) im Vorhofohr lokalisiert sind. Deshalb entfernen Chirurginnen und Chirurgen schon seit vielen Jahrzehnten bei offenen Herzoperationen das Vorhofohr, um es als Emboliequelle zu eliminieren. Als minimal-invasive Alternative wurden seit 10–20 Jahren verschiedene Schirme für den perkutanen Verschluss des Vorhofohrs entwickelt. Die Voraussetzungen, Techniken und Resultate sollen im folgenden Artikel besprochen werden.
