Abstract

MUERTE CEREBRAL es la expresión popular más común relacionada a la MUERTE ENCEFÁLICA o etapa final de la cascada isquémica, es decir, el proceso de la falla energética celular, donde los leucotrienos y la extravasación de proteína sérica y otras moléculas al espacio extracelular producen el edema vasogénico, la disminución de ATP, fosfocreatina, oxidación fosforilativa que va con acidosis tisular que inhibe la fosforilación mitocondrial, compremete la microcirculación, desnatura las proteínas y el ácido nucleico, incrementa los radicales libres, aumenta el ácido láctico, lactatos, que a su vez producen edema citotóxico, alterándose la homeostasis, de los canales y disminuyendo el citosol del calcio que es secuestrado en células del RES y mitocondrias; los edemas vasogénico y citotóxico incrementan los osmoles idiogénicos que incrementan la osmolaridad tisular, el agua intracelular, radicales libres y excitotoxicidad con falla de la barrera hemato-encefálica tisular, el agua intracelular, radicales libres y excitotoxicidad con falla de la barrera hemato-encefálica (BBB) y severa disminución de neurotransmisores, el cerebro sin oxígeno produce la euforia, risa y alucinaciones visuales seguidas de la percepción de luz al final del túnel, debido a la concentración del campo visual cortical, por liberación de sustancias dopaminérgicas y endorfina.
