Evaluación del cuestionario de calidad de vida relacionada con la salud

Abstract

Introducción: la calidad de vida relacionada con la salud (CVRS) se ha convertido en una medida de eficacia en los estudios clínicos de pacientes portadores del virus de inmunodeficiencia humana (VIH) a partir de la efectividad de los tratamientos antirretrovirales, llegando a considerarse al VIH como una enfermedad crónica. Objetivos: evaluar un instrumento para medir la CVRS (FAHI) y su relación con una serie de variables clínicas, psicológicas y sociodemográficas de una población de personas viviendo con VIH. Material y método: se trata de un estudio analítico-descriptivo de la CVRS en pacientes portadores de VIH de la unidad de medicina preventiva del Centro de Salud del Sindicato Médico del Uruguay (CASMU). Se estudió la validez y confiabilidad de los instrumentos utilizados. Resultados: el FAHI mostró una adecuada consistencia interna. La presencia de bajo recuento de linfocitos CD4, un mayor nivel de carga viral para VIH, y/o una estadificación de etapa sida, según la clasificación del Centers for Disease Control and Prevention (CDC), no se asociaron con una peor percepción de la CVRS. La presencia o no de sintomatología tendría un peso mayor en la CVRS. El recibir tratamiento antirretroviral se relacionaba con un mejor bienestar funcional tanto en hombres como en mujeres; evidenciándose en las mujeres una peor percepción del bienestar físico. Conclusión: el FAHI mostró una adecuada consistencia interna y validez acorde al instrumento original. Se destaca la importancia de la valoración de la CVRS a la hora de decidir inicio o cambios de tratamiento antirretroviral (TARV). Con respecto al malestar psicológico, el Symptom Check List 90-R (SCL90-R) correlacionó positivamente con la percepción de la CVRS.
