BACTERIEMIA NOSOCOMIAL ASOCIADA CON CATETERISMO VENOSO CENTRAL. FACTORES DE RIESGO EN CUIDADOS INTENSIVOS PEDIÁTRICOS

Abstract

Las infecciones del torrente sanguíneo (bacteriemias) constituyen una causa importante de morbilidad y mortalidad hospitalarias. Objetivo: Caracterizar epidemiológicamente los episodios de bacteriemia nosocomial asociada con cateterismo intravascular en cuidados intensivos pediátricos. Métodos: Se realizó un estudio prospectivo analítico en la sala de cuidados intensivos del Hospital Pediátrico “José Luis Miranda”, desde enero de 2003 a diciembre de 2007, que incluyó a 453 pacientes. Se determinaron tasas, densidad de incidencia y se analizaron factores de riesgos, estadía y mortalidad. Resultados: 96 pacientes desarrollaron 122 episodios de bacteriemia y 90 (74 %) tuvieron criterios microbiológicos. Los factores de riesgo asociados fueron: tener cateterismo múltiple, permanencia con el catéter superior a 7 días, alimentación parenteral, ventilación mecánica prolongada, transfusiones previas e intervenciones quirúrgicas. Predominaron los aislamientos de estafilococos coagulasa negativo en 33 (36,7 %). La estadía previa a la inserción del catéter fue de 4,0 ± 9,4 días para los sanos y 11, 6 ± 24,6 días en los enfermos (p = 0,000); la estadía definitiva fue de 56,1 ± 62,4 días en los enfermos versus 24,6 ± 31,7 días en los sanos (p = 0,000). La mortalidad de ese grupo fue superior (26 %). Conclusiones: Existen múltiples factores dependientes e independientes sobre los que deben volcarse las acciones para prevenir y disminuir la mortalidad por bacteriemias asociadas con catéteres en niños ingresados en cuidados intensivos.
