Peritonitis biliar. Falla renal aguda

Abstract

Presentamos un caso de peritonitis biliar con falla renal aguda, en la evolución de una colecistitis aguda litiásica y coledocitis litiásica supurada, con cuadro peritoneal. El paciente ingresó como srndrome ictérico coledociano; se realizó el diagnóstico correcto y fue intervenido luego de breve "reanimación", cumpliendo como táctica el tratamiento de la lesión causal primaria y de la peritonitis biliar existente. Se prosiguió en el postoperatorio la terapia del síndrome metabólico humoral y de la falla renal aguda existente; uso del diurético osmótico "Manitol". La evolución fue satisfactoria. Presentamos un análisis del síndrome bioquímico, con comentario sintético del problema. En nuestro caso se realizó la intervención quirúrgica en plena falla renal aguda, con tratamiento concomitante del problema metabólico.
