Enfermedad de Paget de la mama

Abstract

Los caracteres histopatológicos de la enfermedad descrita por Paget en 1874, son los siguientes. En la piel del pezón y aréola, la presencia de las grandes células claras de Paget, intraepidérmicas. Dicha célula es neoplásica y maligna. Existe en los conductos terminales del pezón una proliferación epitelial atípica intraductal, que también es carcinomatosa. Finalmente, si el tiempo es suficiente, la presencia de un carcinoma profundo mamario, de tipo común. Para un grupo de autores la célula de Paget procede de un cáncer de los conductos del pezón y ha invadido la epidermis de la zona. Para otro grupo, dicha célula es autóctona de la epidermis y en consecuencia coexiste con la lesión de los conductos y eventualmente con el cáncer mamario profundo. Esta última tendencia es la que predomina actualmente. La lesión cutánea es en realidad un carcinoma "in situ" o intraepitelial.
