FACTORI DE RISC ASOCIAȚI INFECȚIILOR OTICE CU STREPTOCOCCUS PNEUMONIAE

Abstract

Streptococcus pneumoniae, cauză majoră a infecţiilor bacteriene, este unul dintre cei mai frecvenţi agenţi patogeni implicat în otita medie acută la copil. Obiectiv: evaluarea sensibilitaţii tulpinilor de Streptococcus pneumoniae şi identifi carea factorilor de risc asociaţi, cât şi infl uenţa acestora asupra terapiei şi evoluţiei bolii. S-a realizat un studiu retrospectiv, pe 147 copii diagnosticaţi cu otită medie (cu vârste între 4 luni şi 7 ani), internaţi în clinica II Pediarie. Rezultate: din cei 147 copii cu otită medie, examinarea microbiologică (realizata doar în 97 cazuri) a pus în evidenţă prezenţa Streptoccoccus pneumoniae în 85 cazuri, majoritatea tulpinilor având un nivel crescut de rezistenţă la antibiotice. Factorii de risc, în ordinea frecvenţei: antibioticoterapia frecventă şi prelungită, frecventarea grădiniţelor, internări anterioare pentru otite, expunerea la aeroelergeni, fumat pasiv, hipogamaglobulinemia, prezenţa refl uxului gastroesofagian, statusul socioeconomic precar. Durata terapiei a fost în medie de 10 zile, evoluţia fi ind mai lentă în cazurile infecţiilor cu tulpini multirezistente, la cei cu multiple antibioterapii în antecedente, disimunităţi sau cu expunere importantă la noxe. Concluzii: Tratamentul şi evoluţia otitelor pneumococice sunt infl uenţate atât de factorii de risc asociaţi, cât şi de gradul de sensibilitate al tulpinilor implicate. 57 de tulpini multirezistente din 97 studiate reprezintă o cifră importantă, un semnal privind consecintele administrarii abuzive a antibioterapiei şi subliniază importanţa vaccinării specifi ce în special la copii cu risc.
