Encefalitis Autoinmune y su relación con Lupus Eritematoso Sistémico

Abstract

Las encefalitis autoinmunes son un grupo de enfermedades inflamatorias producidas por una respuesta inmunológica contra determinadas proteínas de la superficie neuronal. Clínicamente pueden afectar cualquier función neurológica, desde funciones mentales, hasta funciones de procesamiento y producción de lenguaje, movimiento, memoria, e incluso provocar crisis epilépticas y disfunción autonómica. Hasta el momento se han demostrado varios tipos de encefalitis autoinmune, cada una con sus propias características fisiopatológicas. La encefalitis asociada a anticuerpos anti N-Metil-D-aspartato (NMDA) es una de las más comunes y mejor estudiadas. El receptor NMDA presenta 4 subunidades 2 GluN1 y 2 GluN2; una de estas interviene en la patogénesis del lupus eritematoso sistémico neuropsiquiátrico a través de una reacción cruzada entre los anticuerpos anti DNA de doble cadena y el receptor NMDA. Se han reportado casos aislados de pacientes con fotosensibilidad que precede a manifestaciones neuropsiquiátricas, o presentación conjunta con linfopenia, injuria renal con sedimento activo y proteinuria, sin encontrarse un patrón clínico específico.
