Prevalencia y factores asociados a injuria renal aguda en adultos mayores hospitalizados

Abstract

Introducción: La injuria renal aguda (IRA) es un trastorno muy frecuente, actúa como factor de morbimortalidad en adultos mayores. Es un problema de salud pública asociado a varios factores, algunos modificables. Métodos: Estudio analítico de corte transversal realizado en Cuenca en el servicio de emergencia del HVCM desde marzo 2018 hasta marzo 2020. El muestreo fue probabilístico, valoró 200 adultos mayores. Se utilizaron los criterios KDIGO (Kidney Disease: Improving Global Outcomes) para determinar y clasificar IRA. Los datos fueron analizados con el programa SPSS v.20.0, utilizando la razón de prevalencia (RP) para medir asociación, con IC 95% y con significancia estadística P < 0.05. Resultados: La media de edad fue 80 años ± 8.37, predominó el género femenino y la mayoría tuvo un nivel de instrucción primaria. La prevalencia de IRA fue 33%. Cuatro de cada diez adultos mayores presentó estadio 3 de IRA y más de la mitad presenta hipertensión arterial y consumo de nefrotóxicos. Los factores asociados fueron: sepsis (RP 2.27; IC95% 1.58–3.27; P =0.0002), deshidratación (RP 1.73; IC95% 1.18–2.54; P =0.006) y diabetes mellitus (RP 1.58; IC95% 1.07–2.34; P =0.0181). Conclusión: Más de un tercio de adultos mayores hospitalizados presentaron IRA, predominó el estadio 3 de KDIGO y estuvo asociado a: sepsis, deshidratación y diabetes mellitus.
