Pulpotomia o Pulpectomia: Éxito clínico y radiográfico en dientes temporales

Abstract

Objetivo: Comparar la tasa de éxito clínico y radiográfico entre PE y PO en dientes temporales. Metodología: Estudio cuasi-experimental. Se incluyó a todos los pacientes con diagnóstico de terapia pulpar y fueron tratados entre agosto 2013 hasta septiembre de 2014 en la Clínica de Odontopediatría de la Facultad de Estomatología de la Universidad Autónoma de San Luis Potosí (UASLP). Un total de 107 sujetos (a los 256 órganos dentales de los 158 pacientes (128 PE y 128 PO). Los tratamientos fueron realizados según la normativa de la Asociación Americana de Odontopediatría. Se evaluó el éxito clínico y radiográfico a los 7, 15, 30, 90, 180 y 360 días post-tratamiento mediante la escala de Zurn-Seale. Las tasas de éxito se compararon con regresión logística ajustada por sexo y edad. Resultados: La tasa de éxito clínico para pulpotomía (91.5%) y de pulpectomía (87.5%) y de éxito radiográfico para pulpotomía (81.4%) y de pulpectomía (83.3%). Después de 360 días, no presento diferencias significativas entre la pulpectomía y pulpotomía, clínicamente OR= 1.54 (0.44-5.40) p= 0.49 y radiográficamente OR= 0.87 (0.32-2.37) p=0.79. Discusión: Ambas terapias pulpares son adecuadas para dientes temporales con diagnóstico de enfermedad pulpar, ya que de acuerdo a los resultados obtenidos no se observa diferencia entre ambos tratamientos.
