Profil Lipid pada Fase Akut Demam Berdarah Dengue

Abstract

Lipid dan lipoprotein bisa menjadi penentu derajat manifestasi infeksi dengue pada fase awal. Telah dilakukan beberapa penelitian mengenai faktor tersebut, namun hasilnya masih kontroversi.Penelitian ini bertujuan untuk mengetahui hubungan profil lipid dengan DBD.Suatu case-control study yang dilakukan dari bulan Maret-September 2011 di tiga rumah sakit dan satu puskesmas di Palembang. Kelompok kasus adalah subjek dengan DBD, dan kontrol adalah subjek non DBD (jumlah dua kali kasus) yang disesuaikan berdasarkan usia dan jenis kelamin. Sampel darah diambil pada hari ketiga demam. Analisis data menggunakan T-test, uji ANOVA atau Kruskal Wallis, chi-squared atau Fisher, dan analisis korelasi Pearson. Analisis bivariat menggunakan SPSS 18.Sebanyak 54 subjek yang terdiri dari 18 subjek DBD sebagai kelompok kasus, dan masing-masing 18 subjek DD dan OFI sebagai kelompok kontrol. Rerata trigliserid pada infeksi dengue lebih tinggi dibanding non dengue, sedangkan rerata kolesterol, HDL dan LDL pada DBD lebih rendah dibanding non DBD. Terdapat perbedaan bermakna rerata trigliserid (p: 0,041; Kruskal Wallis) dan kolesterol (p:0,013; ANOVA). Uji chi-square menunjukkan adanya hubungan antara kadar lipid dengan manifestasi DBD. Terdapat hubungan yang bermakna antara penurunan kolesterol (OR: 2,227; p: 0,041; Fisher) dan HDL (OR: 3,250; p: 0,003; chi-squared) dengan risiko terjadinya DBD pada kelompok DBD dan OFI, dan terdapat hubungan yang bermakna antara penurunan HDL dengan risiko DBD pada kelompok DBD dan non DBD (OR: 2,600; p:0,043; Fisher).Perubahan kadar lipid merupakan faktor risiko manifestasi DBD.   Kata kunci: profil lipid, DBD, DD, OFI, penelitian case-control
