Impacto de la cirugía bariátrica en pacientes con diabetes mellitus tipo 2

Abstract

Introducción: La obesidad es la principal causa del aumento de la diabetes mellitus tipo 2 (DM2). La cirugía bariátrica (CB) ha demostrado ser beneficiosa en el control metabólico y de la comorbilidad asociada a la obesidad en pacientes con DM2. Objetivo: Evaluar el impacto de la CB en pacientes obesos con DM2 y comorbilidad: presión arterial (PA) y dislipidemia. Método: Estudio observacional, retrospectivo, de una cohorte de pacientes del Programa de Obesidad y Cirugía Bariátrica del Hospital Maciel con DM2, intervenidos con CB durante el período 2015-2019, con controles posoperatorios en 3-6 y 12 meses. Se estudiaron el índice de masa corporal (IMC), la glucemia de ayuno (GA), la hemoglobina glucosilada (HbA1c), la PA y el lipidograma. Se evaluó el porcentaje de remisión de la DM2. Resultados: Se analizaron 72 pacientes. La edad promedio fue de 46.8 años y el 79.6% eran mujeres. Se evidenció una reducción estadísticamente significativa del IMC, la GA, la HbA1c, la PA y los triglicéridos al año del posoperatorio. En el 76.4% se observó remisión de la DM2. Comparando la gastrectomía vertical en manga y el bypass gástrico no se encontraron diferencias significativas en las variables estudiadas. Conclusiones: La CB es efectiva en el control de la DM2 y su comorbilidad, y debe ser considerada como una opción terapéutica.
