Linfoma de hodgkin del adulto.

Abstract

Introducción: Linfoma de Hodgkin (LH), representa el 14% de las malignidades linfoideas. La distribución de los subtipos histológicos varía de un país a otro; en Estados Unidos la esclerosis nodular es predominante y la celularidad mixta es más frecuente en América Central y Sudamérica. La tasa de cura excede el 80% con tratamiento, sin embargo en estadío avanzado, factores pronósticos pueden ayudar a identificar pacientes en riesgo de recaída que pueden ser incorporados a tratamientos intensificados. Materiales y Métodos: Se realizó una revisión de10 años de LH del adulto en el Instituto Oncológico Nacional-SOLCA de Guayaquil, desde enero de 1998 a diciembre 2007. Resultados.- Se encontró 142 casos. Esclerosis nodular fue el subtipo más frecuente (51.4%). 45.8% de pacientes se presentaron con síntomas B, 4.9% con localización extraganglionar, 8.5% con infiltración a médula ósea. La tasa de Respuesta Completa (RC) a ABVD en este estudio llega al 62.7%; sin embargo cuando la quimioterapia se administró con intensidad adecuada, RC superó el 90%. Hubo 71,8% de pacientes desaparecidos, ya sea del tratamiento o del seguimiento, por falta de un adecuado programa de control. Conclusiones: Es urgente hacer conciencia que el retraso en la ayuda en el momento apropiado es perder la oportunidad de curar a un paciente, lo que conlleva a un mayor gasto de recursos.
