LITIASIS INTRAHEPÁTICA

Abstract

Introducción: La litiasis intrahepática es poco frecuente en países occidentales, generalmente debuta con un síndrome colestásico que, asociado a complicaciones necesita resolución quirúrgica urgente. Caso Clínico: Paciente masculino, 56 años, diabético, colecistectomizado, consulta por alza térmica, coluria, acolia y dolor abdominal a nivel epigástrico con irradiación a hipocondrio derecho. Presenta ictericia, leucocitosis con neutrofilia, PCR >100, hiperbilirrubinemia (4.51) a expensas de la directa (3.16), TGP 210, TGO 86 y GGT 362. La Ecografía abdominal reportó dilatación de la vía biliar intrahepática y coledocolitiasis. Resultados: Tras un diagnóstico de coledocolitiasis más colangitis, se inició antibiótico terapia, se extrajeron varios litos mediante CPRE, dejando litos intra-hepáticos residuales. La colangio-resonancia reportó litos a nivel de conducto biliar intra-hepático izquierdo y atrofia de lóbulo hepático izquierdo, por lo cual se realizó la hepatectomía de los segmentos II y III con litos palpables en su interior. El reporte anatomo-patológico correspondió a colangiocarcinoma intrahepático multicéntrico moderadamente diferenciado. Conclusiones: La hepatectomía resuelve definitivamente la hepatolitiasis no complicada, y permite hasta un 79% de sobrevida a 5 años en casos de litiasis intrahepática asociada a colangiocarcinoma; sin embargo, la asociación de un diagnóstico tardío más irresecabilidad, habitualmente sitúa al colangiocarcinoma intrahepático como un tumor de mal pronóstico.
