Abstract

La exploración quirúrgica sistemática para las heridas abdominales penetrantes es en el Hospital Maciel el patrón de referencia diagnóstico y terapéutico. Con el objetivo de analizar la tasa de cirugías innecesarias (incluyendo en ellas tanto las que no logran identificar lesiones, como aquellas que habiéndolas identificado no cumplieron ningún rol terapéutico), se estudió en forma retrospectiva una serie de 238 pacientes operados por trauma a lo largo de 2 años y cinco meses (octubre de 2001 a marzo de 2004) en el Departamento de Emergencia del Hospital Maciel. Se efectuaron un total de 126 laparotomías exploradoras por heridas penetrantes de abdomen. Correspondieron a heridas punzocortantes 67, de las cuales 22 (33%), fueron laparotomías innecesarias. De las 59 laparotomías para tratar heridas por proyectiles de arma de fuego, fueron innecesarias 11 de ellas (18,6%).
