Función cardioprotectora del Remifentanilo

Abstract

La causa principal mortalidad perioperatoria es cardiovascular; lo cual se debe a varios factores: alta prevalencia de enfermedad cardiovascular en la población, fenómenos de isquemia-reperfusión en cirugía cardiaca, activación protrombótica secundaria al dolor y estrés quirúrgico, disbalance entre consumo y aporte miocárdico de oxígeno a favor del primero; y trombosis coronaria secundaria al desarrollo de fuerzas de cizallamiento intraluminar (1). El acondicionamiento es un mecanismo innato de protección frente a un evento isquémico potencialmente letal. En la Figura 1 se muestra la clasificación del acondicionamiento (2,3). Los estímulos que lo desencadenan liberan varias sustancias, de las cuales las más reconocidas son los autacoides: adenosina, opioides y bradiquinina. La unión de estas sustancias con su receptor produce la activación de nuevas vías moleculares. Este proceso conduce a la aplicación clínica de medicamentos y al llamado acondicionamiento farmacológico. El Remifentanilo podría ser uno de los fármacos involucrados en este posible efecto cardioprotector, como veremos más adelante.
