Abstract

[ES] En la actualidad, el aumento de las resistencias antibióticas representa un desafío a nivel global para la salud pública, destacando entre ellas las Enterobacterias y Pseudomonas aeruginosa productoras de carbapenemasas que confieren resistencia a gran parte de los antibióticos disponibles. En estos casos y, en particular, en las metalobetalactamasas (carbapenemasas de clase B), las escasas opciones terapéuticas provocan que no sea siempre suficiente el tratamiento en monoterapia. Por lo que el estudio del efecto sinérgico entre diversos antibióticos cobra especial relevancia. La sinergia se da cuando dos antibióticos interactúan positivamente potenciando su efecto antimicrobiano. Son numerosos los métodos para estudiar el efecto sinérgico entre dos antibióticos. El objetivo de este trabajo es evaluar la concordancia y rendimiento de cuatro métodos para el demostrar in vitro el efecto sinérgico entre Ceftazidima-Avibactam y Aztreonam en cepas de Enterobacterias y Pseudomonas aeruginosa productoras de carbapenemasas de clase B. Para ello se utilizaron cepas criopreservadas de Pseudomonas aeruginosa y diversas especies de Enterobacterias aisladas de pacientes en el Servicio de Microbiología del Hospital Clínico Universitario de Valencia. Estas se analizaron, a partir de colonia bacteriana aislada, por cuatro métodos: , concretamente:1. el método de referencia de tiras E-test cruzadas (TC) y cálculo del iCIF; 2. resensibilización de Aztreonam (ATM) mediante tiras E-test paralelas (TP); 3. mMétodo de disco de ATM junto a tira E-test de CZA para observar el aumento de halo de ATM y/o la D invertida (TD); y 4. Método de elución en medio Cerebro-Corazón con discos de CZA y ATM (ED), determinando por MALDI-TOF la ausencia o presencia de crecimiento bacteriano con base en el resultado de la identificación.
