Karakteristike humanih papilomavirusa (HPV), HPV-infekcije i anti-HPV cjepiva

Abstract

Ljudski papilomavirusi (engl. human papilloma virus, HPV) svrstani su u obitelj Papovaviridae, skupinu malih, neovijenih DNK virusa oblika ikozaedra. Naziv im je izveden od latinske riječi papilla (bradavica) i grčke riječi oma (tumor). Kapsida promjera 55-60 nm sastavljena im je od 72 podjedinice – kapsomere, a veliki protein kapside (L1) tvori 80-90% svih proteina kapside. Genom HPV-a dijeli se na tri područja: područje L koje upravlja sintezom strukturnih proteina virusa, područje E sa zapisima tvorbe proteina odgovornih za umnožavanje virusa i onkogeni potencijal virusa, te područje R koje sadržava zapise za regulacijske proteine koji upravljaju procesima umnožavanja virusa. Ljudski papilomavirusi se na temelju podudarnosti nukleotidnih nizova dijele u različite genotipove, a danas ih je poznato više od 130. Svaki novoizdvojeni virus koji se razlikuje u više od 10% nukleotidnih nizova u području L1 od već poznatih genotipova virusa dobiva novi broj. Većina infekcija HPV-om je asimptomatska, prolazna i virus spontano nestaje, no ponekad infekcija može rezultirati postupnom tvorbom različitih tumora pločastog epitela. Tvorba tumora zbiva se u tri faze, u prvoj stanicu inficira HPV, u drugoj uz HPV važni su i različiti drugi faktori (imunosupresija, zračenje, kemijske tvari, cigaretni dim itd.), dok u trećoj dolazi do ugradnje virusne DNK u DNK stanice domaćina. U dobroćudnim tumorima brojne kopije virusne DNK prisutne su u obliku episoma – izvankromosomske DNK, dok je u zloćudnim tumorima virusna DNK uvijek ugrađena u genom ljudske stanice. Sklonost ugradnje u genom ljuske stanice pokazuju tzv. genotipovi visokog rizika, najviše HPV 16, dok genotipovi niskog rizika nemaju sklonost ugradnje genoma u genom inficirane stanice. Od 1995. godine HPV 16 i HPV 18 drže se dokazanim karcinogenim čimbenicima prvog stupnja za čovjeka. Dobroćudni tumori pločastog epitela, šiljasti kondilomi i planocelularni papilomi grkljana uzročno su povezani s infekcijom niskorizičnim genotipovima 6 i 11. Visokorizični genotipovi HPV-a, posebice genotipovi 16 i 18, otkrivaju se u gotovo 100% uzoraka karcinoma vrata maternice, a noviji radovi pokazuju važnost genotipova 16 i 18 u etiopatogenezi orofaringealnog karcinoma. Nova cjepiva protiv HPV-infekcije pružaju novu mogućnost u prevenciji HPV-infekcije i njezinih posljedica (četverovalentno anti- HPV 6, 11, 16 i 18 cjepivo Gardasil i dvovalentno anti- HPV 16 i 18 cjepivo Cervarix). Dobivena su genetskim inženjerstvom i sastoje se samo od virusnih kapsidnih proteina, a kako ne sadržavaju virusnu DNK ne mogu izazvati HPV-infekciju. Cjepiva nemaju nikakvog terapijskog učinka niti se mogu upotrebljavati u liječenju HPV-infekcije i bolesti povezanih s HPV-infekcijom.
