Molekularna dijagnostika hiperkolesterolemije

Abstract

Istraživanja hiperkolesterolemije ukazuju da katabolizam lipoproteina male gustoće (LDL) ovisi o funkcionalnoj djelotvornosti LDL-receptora (LDLR) i strukturnoj osobitosti apolipoproteina B (ApoB). Približno jedna od 500 osoba u općoj populaciji je nositelj defektnog LDLR gena za kojeg je već poznato oko 200 različitih mutacija. Gotovo u istoj proporciji prisutna je hiperkolesterolemija kao posljedica mutacije gena za ApoB. Cilj ovog rada je istraživanje molekularnog mehanizma hiperkolesterolemije u Hrvatskoj. Određivanje funckionalne aktivnosti LDLR provedeno je metodom fluorescentne protočne citometrije u bolesnika čije su vrijednosti ukupnog kolesterola (ł 6, 7 mmol/L) i LDL-kolesterola (ł 4, 9 mmol/L), te isključenje sekundarnih čimbenika hiperkolesterolemije ukazivali na genetsku osnovu bolesti. Prisustvo molekularnih promjena ispitano je metodom PCR-SSCP (konformacijski polimorfizam jednolančane DNA) za 5 eksona LDLR gena u kojih su mutacije najčešće prisutne. Rezultati pokazuju da su u 5% bolesnika prisutne promjene LDLR gena koje utječu na smanjenu funkcionalnu aktivnost LDLR proteina. Pored kliničke primjene rezultati ovog rada doprinose i boljem poznavanju patogeneze kroničnih bolesti vezanih uz metabolizam lipoproteina.
