Ropivacaina versus bupivacaina en anestesia peridural para histerectomía abdominal

Abstract

Introducción: La ropivacaína es un isómero con menos efectos cardiotóxicos y menor bloqueo motor, efecto vasoconstrictor y dolor a la inyección. Objetivos: Determinar la eficacia de la ropivacaína por vía epidural en histerectomía abdominal. Método: Se estudiaron 80 pacientes, todas ASA I-II, divididas en dos grupos: Grupo R (estudio) 40 pacientes se les administró 20 ml de ropivacaína al 1 %. Grupo B (control) igual al anterior pero se empleó la bupivacaína al 0,5%. Se evaluó el bloqueo sensitivo y motor. Se observaron las complicaciones hemodinámicas intraoperatorias y se evaluó la analgesia residual postoperatoria en las primeras 6 horas por una Escala Análoga Visual. Resultados: A los 10 min todas las pacientes presentaron sensación disminuida o de objeto romo. A los 15 min no hubo percepción en 85 % del grupo R y en 27 % del B. A los 30 min. Existió bloqueo sensitivo en todas las pacientes. El bloqueo motor no tuvo diferencias estadísticamente significativas. A los 30 min no flexionaron las rodillas el 62.5 y 85% del grupo R y B respectivamente. El grupo R reportó 32 % de complicaciones hemodinámicas y 75 % en el B, correspondiéndole a la hipotensión arterial el 22.5 y 55 % respectivamente. En la 2da hora descendieron los niveles de analgesia del grupo B. En la 3era hora el grupo R exhibió 32.5 % de analgesia moderada y en el B 35 % con 15 % de analgesia no satisfactoria. A la 6ta hora este grupo presentó 57.5 % de analgesia no satisfactoria y 10 % los del R (p< 0.05). Conclusiones: La ropivacaína ofrece un bloqueo sensitivo más rápido y motor cualitativamente menor y brinda más analgesia postoperatoria La hipotensión arterial es mas frecuente con la bupivacaína.
