Bupivacaina por vía subaracnoidea en la operación cesárea

Abstract

Introducción: La anestesia regional es la técnica de elección en la operación cesárea por sus beneficios para el binomio madre-feto. Objetivo: Evaluar la calidad anestésica de la combinación de bupivacaína 0,5% - fentanyl - morfina y lidocaína 5% - morfina, en la anestesia espinal para operación cesárea. Método: Se realizó un estudio clínico, descriptivo, prospectivo, en el Hospital Universitario "Abel Santamaría", de Pinar del Río, de septiembre 2005 a mayo 2006, en una muestra de 140 pacientes obstétricas, anunciadas para operación cesárea, con clasificación ASA I-II. Talla superior a 1.50 metros. Existieron dos grupos de tratamiento, cada uno con 70 pacientes. El grupo controles recibió lidocaína 5 % 60 mg y morfina liofilizada 0.1 mg y el grupo de casos recibió bupivacaína 0.5 % 7.5 mg, fentanyl 10 µg y morfina liofilizada 0.1 mg. Resultados: Existió mejor calidad anestésica en el grupo de casos, con diferencias significativas. El tiempo requerido para la altura del dermatoma T6 y regresión total del bloqueo sensitivo fue significativamente superior en el grupo de casos, donde el bloqueo motor de las extremidades inferiores fue menor. La hipotensión intraoperatoria fue la complicación más frecuentes en ambos grupos. La calidad analgésica fue buena en 92.9 % del total de las pacientes. Todas las pacientes presentaron prurito. Conclusiones: La utilización de combinaciones de dosis bajas de bupivacaína 0.5% - fentanyl - morfina y lidocaína 5% - morfina, ofrecen condiciones quirúrgicas adecuadas para la realización de operación cesárea.
