PARTICULARITĂȚILE EVOLUȚIEI HIPERTENSIUNII PULMONARE LA PACIENȚII CU INSUFICIENȚĂ CARDIACĂ ISCHEMICĂ PE PARCURSUL A 12 LUNI DUPĂ REVASCULARIZARE MIOCARDICĂ

Abstract

Hipertensiunea pulmonară HTP secundară bolilor cordului stâng este frecventă și are un impact important asupra progresiei bolii, morbidității și mortalității. Scop: identificarea particularităților evoluției HTP la pacienții cu insuficiență cardiacă ischemică pe parcursul a 12 luni după revascularizare miocardică MRev prin by-pass coronarian sau angioplastie coronariană percutană. Metode: Cercetarea a inclus 281 pacienți supuși MRev vârsta medie=63,2plusmn;9,7ani, 76,5%- bărbați, supravegheați timp de 12,6plusmn;1,6luni. Pacienții au fost divizați în funcție de probabilitatea ecocardiografică HTP PrHTP în 3 grupuri: PrHTP mică 117 pacienți, PrHTP intermediară 74 bolnavi, PrHTP înaltă 90 pacienți. Rezultate. Prevalența HTP la 12 luni după MRev a consitutuit 69,7%, majorându-se cu 10% comparativ cu perioada inițială. S-a atestat o evoluție negativă a PrHTP pe parcursul 1 an după MRev, manifestată prin creșterea gradului de PrHTP la 42,8% dintre bolnavii cu PrHTP mică inițial și 54,1% dintre cei cu PrHTP intermediară. Velocitatea maximă a regurgitării tricuspide și presiunea sistolică în artera pulmonară nu s-au modificat semnificativ pe parcursul a 12 luni. Evoluția negativă a PrHTP a fost determinată de dinamica negativă a semnelor ecocardiografice adiționale sugestive pentru HTP: aria atriului drept +2,2plusmn;0,4mm, timpul de accelerare în tractul de ejecție al ventriculului drept -7,6plusmn;1,2ms, diametrul venei cave inferioare +0,55plusmn;0,2mm. Pe parcursul a 12 luni după MRev au decedat 5 pacienți, toți fiind cu PrHTP înaltă p0.01, iar rata spitalizărilor în acest grup a fost semnificativ mai mare PrHTP înaltă ndash; 57,1%, PrHTP intermediară ndash; 43,2%, PrHTP mică ndash; 38,7%, p0.05. Concluzii: Prevalența HTP la 12 luni după MRev a consitutuit 69,7%, majorându-se cu 10% comparativ cu perioada inițială. S-a remarcat o evoluție negativă a PrHTP în acest interval de timp. Pacienții cu PrHTP înaltă și intermediară au avut o rată mai mare de spitalizări pe parcursul a 12 luni după MRev, iar toți pacienții decedați au avut PrHTP înaltă.

Легочная гипертензия ЛГ ассоциированная с поражениями левых отделов сердца встречается часто и оказывает важное влияние на заболеваемость и смертность. Целью нашей работы было выявить особенности развития ЛГ у больных с ишемической сердечной недостаточностью в течение 12 месяцев после реваскуляризации миокарда МРев методом коронарного шунтирования или коронарной ангиопластики. Методы: 281 пациент перенесшие МРев были включены в исследование средний возраст ndash; 63,2plusmn;9,7 года, 76,5% ndash; мужчины. Продолжительность наблюдения ndash; 12,6plusmn;1,6 месяцев. Больные были разделены в зависимости от эхокардиографической вероятности ЛГ ВерЛГ на 3 группы: низкиая ВерЛГ 117 пациентов, средняя ВерЛГ 74 пациента, высокая ВерЛГ 90 пациентов. Результаты. Распространенность ЛГ через 12 месяцев после МРев составила 69,7%, увеличившись на 10% по сравнению с исходным периодом. Мы заметили отрицательную эволюцию ВерЛГ в течение 1 года после МРев, проявляющуюся увеличением степени вероятности ЛГ у 42,8% пациентов с исходно низкой ВерЛГ и у 54,1% пациентов со средней ВерЛГ. Максимальная скорость регургитации трикуспидального клапана и систолическое давление в легочной артерии не изменились значительно в течение 12 месяцев. Негативная эволюция ВерЛГ определяется отрицательной динамикой дополнительных эхокардиографических признаков, указывающих на ЛГ: площадь правого предсердия +2,24plusmn;0,42 мм, время ускорения в легочной артерии -7,6plusmn;1,2 мс, диаметр нижней полой вены +0,55plusmn;0,2 мм. Через 12 месяцев после МРев не выжили 5 пациентов с высокой ВерЛГ, р0,01, а доля госпитализированных пациентов в группе с высокой ВерЛГ была выше высокая ВерЛГ ndash; 57,1%, средняя ВерЛГ ndash; 43,2%, низкая ВерЛГ ndash; 38,7%, р0,05. Выводы. Распространенность ЛГ через 12 месяцев после МРев составила 69,7%, увеличившись на 10% по сравнению с исходным периодом. В течении года наблюдения отмечена отрицательная эволюция ВерЛГ. Пациенты с высокой и средней ВерЛГ были чаще госпитализированы, а у всех умерших пациентов была высокая ВерЛГ.

Pulmonary hypertension PH associated with left heart disease is common and has an important impact on disease progression, morbidity and mortality. We aimed to identify the particularities of PH evolution in patients with ischemic heart failure during 12 months after myocardial revascularization MRev by coronary artery bypass grafting or percutaneous coronary angioplasty. Methods: The research included 281 patients undergoing MRev mean age=63.2plusmn;9.7 years, 76.5%- men. Follow up period - 12.6plusmn;1.6 months. The patients were divided according to the echocardiographic probability of PH PHPr into 3 groups: low PHPr 117 patients, intermediate PHPr 74 patients, high PHPr 90 patients. Results. The prevalence of PH 12 months after MRev constituted 69.7%, increasing by 10% compared to the initial period. We highlighted a negative evolution of PHPr during 1 year after MRev: 42.8% of patients with initially low PHPr and 54.1% of those with intermediate PHPr had a higher degree of PHPr. The peak tricuspid regurgitation velocity and the pulmonary artery systolic pressure did not change significantly during 12 months after the acute event. The negative evolution of PHPr was determined by the negative dynamics of additional echocardiographic signs suggestive for PH: the right atrium area +2,2plusmn;0.4mm, the right ventricular outflow tract acceleration time -7.6plusmn;1.2ms, the inferior vena cava diameter +0.55plusmn;0.2mm. During 12 months after MRev, 5 patients died, all of them had high PHPr p0.01, and the hospitalization rate was significantly higher in this group high PHPr ndash; 57.1%, intermediate PHPr ndash; 43.2%, low PHPr ndash; 38.7%, p0.05. Conclusions: The PH prevalence 12 months after MRev was 69.7%, increasing by 10% compared to the initial period. A negative evolution of PHPr was noted during 1 year follow up. Patients with high and intermediate PHPr had a higher rate of hospitalizations during the 12 months after MRev and all deceased patients had high PHPr.
