Einzelfallorientierung in der Evidenzbasierung der Sprachtherapie* / Case orientation in evidence-based practice of speech language therapy

Abstract

Ein evidenzbasiertes Vorgehen in der Sprachtherapie wird seit vielen Jahren gefordert. Allerdings fehlt es in vielen Bereichen an den erforderlichen Studien und zusätzlich wird die Übertragbarkeit der Hierarchie von Studiendesigns aus der evidenzbasierten Medizin kritisch diskutiert. In dem vorliegenden Beitrag wird die in der Sprachtherapie wesentliche Orientierung am Einzelfall als Ausgangspunkt genommen, um die Eignung verschiedener Studiendesigns für die Evidenzbasierung in der Sprachtherapie zu diskutieren. Hierzu wird zunächst die Einzelfallstudie in ihren Grundzügen vorgestellt und über verschiedene methodische Varianten ihr Potenzial zu einer größeren Verallgemeinerbarkeit der Ergebnisse dargestellt. Der Übergang von Einzelfallstudien zu versorgungsorientierten Gruppenstudien erscheint dabei fließend. Diese methodischen Überlegungen werden anhand von drei Studien aus dem Bereich der Redefluss- und Kommunikationsstörungen konkretisiert. Abschließend werden die methodischen Erkenntnisse aus diesen Studien zusammengeführt und die Implikationen für die Evidenzbasierung in der Sprachtherapie herausgestellt. Since several years there is a claim for evidence-based practice in speech language therapy. However, in many areas there is still a lack of studies. Additionally, some authors argue that the hierarchy of study designs in evidence-based medicine should not simply be transferred to speech language therapy. In this article case orientation that is crucial in speech language treatment is taken as a starting point to discuss the suitability of different study designs for evidence-based speech language therapy. At first, we explain the fundamentals of single subject (case) studies and some methodological variations that have the potential for better generalization. Doing this, we see a fluent transition from case studies to comparative effectiveness research. The methodological considerations are then illustrated by three studies in the field of fluency and communication disorders. Finally, the methodological findings from these studies are taken together to discuss implications for evidence-based speech language therapy.
