Narcisismo como eixo instintivo transversal aos transtornos do cluster B: um conceito neurocognitivo

Abstract

Propõe-se neste artigo que o narcisismo, longe de ser apenas um transtorno de personalidade isolado, constitui um eixo instintivo transversal a todos os transtornos do cluster B. O narcisismo é entendido como um fator de pertencimento, ligado à necessidade de chamar atenção e obter aceitação social, com raízes adaptativas de sobrevivência grupal. Quanto maior a intensidade do narcisismo, maior a disfunção entre o córtex pré-frontal ventromedial (vmPFC), o orbitofrontal (OFC), o córtex cingulado anterior (ACC) e a ínsula, com consequente hiperatividade da amígdala. Essa rede alterada diminui a empatia e aumenta a busca por meios de destaque social. A hipótese sugere que o narcisismo está presente em todos os transtornos do cluster B, diferenciando-se apenas pela intensidade da baixa empatia e pela forma de expressão social.
