Chirurgies cardiaque et carotidienne combinées : Améliorer les résultats

Abstract

Objectifs Le but de cette etude etait d’analyser notre experience de la chirurgie combinee carotidienne et cardiaque au cours des 5 dernieres annees. Methodes Au cours d’une periode de 5 ans (janvier 2002-decembre 2006), 111 patients ont eu une endarteriectomie carotidienne (EAC) et un pontage aorto-coronaire (PAC) (groupe 1), tandis que 1446 patients ont eu une EAC isolee (groupe 2). Les resultats peri-operatoires des deux groupes ont ete compares en utilisant les tests chi-2 et le test exact de Fisher afin d’analyser les deficits neurologiques, les evenements cardiaques et les deces a 30 jours. Les resultats au cours du suivi ont ete analyses en utilisant les courbes de survie de Kaplan–Meier et les deux groupes ont ete compares en utilisant le test du log-rank. Resultats Les deficits neurologiques post-operatoires immediats sont survenus plus frequemment chez les patient du groupe 1 (2,5 vs. 0,4%, p = 0,002), avec une incidence plus elevee d’accidents ischemiques transitoires dans le groupe 1. Neanmoins, il n’y a pas eu de difference concernant l’incidence d’accidents constitues (1% groupe 1 vs. 0,6% groupe 2, p = n.s.). Les taux de mortalite etaient plus eleves dans le groupe chirurgie combinee (3,5 vs. 0,5%, p Conclusion Dans notre experience, l’EAC combinee a la chirurgie cardiaque comporte un risque plus eleve de mortalite peri-operatoire pour les patients ayant une EAC isolee. Chaque fois que possible, l’EAC doit etre combinee avec un PAC sans CEC. L’EAC combinee avec un PAC a cœur battant semble etre la strategie therapeutique de choix chaque fois que possible.
