Abstract

L'anesthesie regionale (AR) represente le gold standard en termes de surveillance neurologique au cours de l'endarteriectomie carotidienne (EAC). Des donnees recentes montrerent que l'EAC sous AR etait associee a un moindre taux de complications postoperatoires. L'objectif de cette etude etait d'evaluer la stabilite hemodynamique et l'utilisation de drogues vasopressives au cours de l'EAC sous AR versus l'EAC sous anesthesie generale (AG). Tous les patients ayant beneficie d'une EAC entre janvier 2005 et Janvier 2006 furent identifies dans notre base de donnees prospective. Les dossiers medicaux papiers et electroniques furent examines. Les donnees de surveillance per-operatoire furent revues retrospectivement. Une hypotension etait definie par une pression arterielle systolique (PAS) 160 mm Hg. La variation de la PA etait definie par la difference entre le chiffre de PAS le plus haut et le chiffre de PAS le plus bas. Une bradycardie etait definie par un rythme cardiaque (RC) inferieur a 60. Les resultats sont exprimes sous la forme de moyenne ± deviation standard. Soixante-douze patients consecutifs furent operes d'une EAC: 25 sous AR et 47 sous AG. Il n'y avait pas de difference sur le plan du RC et de la PA preoperatoire entre les deux groupes. La plupart des patients avaient une stenose carotidienne severe symptomatique (80% sous AR vs. 85% sous AG, non significatif). En per-operatoire, l'AR etait associee a une moindre variation de la PA (60 ± 27 vs. 78 ± 22 mm Hg, p = 0,005), une diminution des taux de bradycardies (5% vs. 63%, p p p = 0,009) et une augmentation du taux d'hypertensions (80% vs. 47%, p = 0,007).
