Abstract

La administración de anticoagulantes en pacientes con cirrosis hepática es controvertida debido a que esta enfermedad ha sido considerada durante mucho tiempo un estado hipocoagulante. Sin embargo, en las últimas dos décadas se ha puesto de manifiesto que la hemostasia mantiene un equilibrio precario en pacientes con cirrosis hepática, pudiendo presentar tanto un fenotipo hipocoagulante como uno protrombótico, y que la anticoagulación en pacientes con cirrosis hepática no se asocia a un mayor riesgo de episodios hemorrágicos ni a una mayor mortalidad cuando dichos episodios se producen. Por otro lado, se ha sugerido además que este potencial estado procoagulante está íntimamente implicado en la progresión de la enfermedad hepática. Así, un reciente ensayo clínico en pacientes con cirrosis hepática descompensada mostró que la administración de dosis profilácticas de enoxaparina, una heparina de bajo peso molecular, no solo prevenía el desarrollo de trombosis portal, sino que también retrasaba la aparición de complicaciones de la enfermedad y aumentaba la supervivencia. En la misma línea, varios estudios experimentales han observado una mejoría de la fibrosis hepática mediante la administración de diferentes anticoagulantes. Todas estas observaciones, por tanto, establecen una base racional para la utilización de terapias anticoagulantes en la cirrosis. Es importante destacar, sin embargo, que la administración de anticoagulantes en todos los estudios experimentales previos se inició antes de que se hubiese establecido una cirrosis, que los efectos sobre la presión portal no fueron evaluados, y que la mejoría de la fibrosis en la mayoría de ellos fue relativamente pequeña...
