Çocuklarda inek sütü proteini alerjisi

Abstract

Cow’ s milk protein allergy is one of the most common allergies in infants. Cow’s milk allargy is a reaction that develops as a result of an immunological responce to any of the cow’s milk proteins, βlaktoglobulin, α-Laktalbumin and casein. Cow’s milk protein allergy can be classified as IgE-mediated, non- IgE-mediated (cellular) and mixed-type reactions. In order to diagnose this disease, various laboratory tests and milk specific IgE measurement and dri tests (prick and atopy patch test), and oral nutrient challenge tests are applied. In the clinical findings, 50-60% of the cases were reported as skin, 50-60% gastrointestinal and 20-30% respiratory system findings. The simplest and cheapest mothod of cow’s milk protein allergy would be to remove milk and foods containing milk proteins. Families should be informed about this issue by giving various trainings. The objective of this review is to investigate the role of nutrition on the growth and develeopment of the cow milk allergy positive children.

İnek sütü proteini alerjisi (İSPA) bebeklerde en çok görülen alerjilerden başında gelmektedir. İnek sütü alerjisi inek süt proteinlerinden olan βlaktoglobulin, α-Laktalbumin, kazeinden herhangi birine karşı oluşan immünolojik yanıt sonucu gelişen reaksiyonlardır. İSPA; IgE aracılı, IgE aracılı olmayan (hücresel) ve karma tip reaksiyonlar olarak sınıflandırılabilir. Bu hastalığın tanısının konulabilmesi için çeşitli laboratuar tetkikleri ile süt spesifik IgE ölçümü ve deri testleri (prik ve atopi yama testi) ve oral besin yükleme testi uygulanmaktadır. Klinik bulgularda olguların %50-60 deri, %50-60 gastrointestinal ve %20-30’u solunum sistemi bulgularını şeklinde belirtilmiştir. İSPA tedavisinin en basit ve ucuz yöntemi süt ve süt proteinleri içeren besinlerin çıkarılması olacaktır. Ailelere bu konuda çeşitli eğitimler verilerek bilgilendirilmelidirler.
