Abstract

La categoria delle lesioni cistiche del pancreas include formazioni a differente morfologia, patogenesi e biologia. Sebbene siano lesioni rare rispetto alle neoplasie solide rappresentano una entità diagnostica importante ed hanno suscitato notevole interesse negli ultimi anni per tre motivi. 1- Sono lesioni in cui, se appropriatamente trattate, la chirurgia rispetto all’adenocarcinoma duttale del pancreas ha effetto curativo; 2- Le moderne tecniche di imaging permettono di identificare cisti molto piccole ed ancora asintomatiche; 3- La ridotta morbilità della chirurgia pancreatica ha permesso una maggiore aggressività e quindi la possibilità anche di studiare queste lesioni. Complessivamente l’approccio terapeutico e la prognosi dipendono da un corretto inquadramento diagnostico. Sebbene tutte le neoplasie pancreatiche, incluso l’adenocarcinoma duttale del pancreas, si possano mostrare clinicamente come lesioni cistiche, per fenomeni di degenerazione emorragica e/o necrotica di una lesione solida, le neoplasie cistiche propriamente dette sono quelle incluse nella classificazione WHO del 1996 (vedi tabella I). L’Organizzazione Mondiale della Sanità divide le neoplasie in base al loro tipo istologico ed al loro comportamento biologico in tumori benigni, borderline e maligni. E’ importante sottolineare come sia il tipo di epitelio e le sue caratteristiche che determinano il tipo di neoplasia e non le dimensioni o la forma della cisti. Le neoplasie cistiche più frequenti sono rappresentate dalle neoplasie mucinose papillari intraduttali (IPMT) (24%), le neoplasie sierose cistiche (10%), le neoplasie mucinose cistiche del pancreas (8%) ed i tumori cistici pseudopapillari (3%).
