Interés farmacológico y terapéutico de colchicina

Abstract

La colchicina es un principio activo que se obtiene principalmente de las semillas y bulbo de la especie Colchicum autumnale y se ha utilizado durante décadas para el tratamiento de la artritis gotosa por ser un antiinflamatorio específico de las artritis microcristalinas producidas por la acumulación de sales de uratos y la Fiebre Mediterránea Familiar (FMF). La colchicina posee además propiedades antimitóticas. Inhibe la mitosis en metafase, impidiendo que se forme el huso mitótico. Posteriormente, los últimos avances en la investigación han permitido poner de manifiesto los mecanismos antiinflamatorios de la colchicina. Este fármaco es capaz de inhibir la adhesión, la movilización y el reclutamiento de neutrófilos, principalmente paralizando los procesos inflamatorios relacionados con ciertas patologías. Por ello, se ha extendido el uso de este remedio natural para tratar otras muchas enfermedades como la enfermedad de Behçet, pericarditis y aterosclerosis, entre otras; incluso se ha observado que puede llegar a ser empleada en el tratamiento de pacientes con covid-19 aunque su efectividad y seguridad en este caso están aún por demostrarse. Sin embargo, debido a su estrecho margen terapéutico, no es difícil que un porcentaje de pacientes presenten intolerancia a este compuesto debido a reacciones adversas principalmente gastrointestinales. Al metabolizarse por la vía del citocromo P450 y la glicoproteína P puede presentar múltiples interacciones con otros fármacos como la claritromicina o estatinas en cuyo caso sería necesario un ajuste de dosis.
