Abstract

As cáries dentárias na dentição decídua continuam a ser um importante problema de saúde. Quando a lesão se estende e envolve a polpa dentária, as técnicas de terapia pulpar são frequentemente utilizadas na abordagem de ambos os dentes, sintomáticos e assintomáticos. Tanto a pulpotomia como a pulpectomia são, em algumas situações, alternativas apropriadas à extracção dos dentes decíduos, retendo o dente na arcada dentária, num estado assintomático, até à sua perda natural durante a transição da dentição decídua para a permanente. Com esta dissertação pretende-se descrever a pulpotomia e pulpectomia em dentes decíduos e os materiais/técnicas que estas implicam, bem como comprovar o sucesso clínico e radiográfico de ambos os tratamentos, justificando a sua utilização na Odontopediatria. Para a realização deste trabalho fez-se uma pesquisa na Medline/Pubmed, no site da Sociedade Portuguesa de Estomatologia e Medicina Dentária e recorreu-se ainda à bibliografia dos artigos relacionados com este tema e revistas científicas. As preocupações relativas a uma possível toxicidade do formocresol e o reduzido efeito antimicrobiano e reabsorção lenta do óxido de zinco eugenol, levaram os investigadores a uma procura de melhores materiais e/ou técnicas alternativas para a realização de pulpotomias e pulpectomias, respectivamente. De acordo com a literatura, as técnicas actualmente existentes apresentam bons resultados, com taxas de sucesso favoráveis ao seu uso na prática clínica. Contudo o material ideal ainda não foi encontrado. Não existem evidências de qual o material ou a técnica mais apropriadas para a realização de uma pulpotomia ou pulpectomia em dentes decíduos. São necessárias mais investigações nesse sentido, assim como mais estudos de elevada qualidade, de modo a permitir uma escolha mais acertada nos cuidados de saúde prestados.

Dental decay in primary teeth remains a major health problem. When decay extends to involve the dental pulp, pulp therapy techniques are often used to manage both symptomatic and symptom free teeth. Pulpectomy and pulpotomy are, in some situations, appropriate alternatives to the extraction of primary teeth, retaining the tooth in the dental arch, in a symptomatic free state, until it is lost naturally during the transition from primary to permanent dentition. This paper aims to describe pulpotomy and pulpectomy in primary teeth and the materials/techniques that they imply, as well as to demonstrate clinical and radiographic success of both treatments, justifying its use in pediatric dentistry. For this work, searches were made in Medline/Pubmed, in the website of the Portuguese Society of Stomatology and Dental Medicine and in the bibliography of articles on this topic and scientific journals. The concerns about a possible formocresol’s toxicity and reduced antimicrobial effect and slow absorption of zinc oxide eugenol, led investigators to search for better alternative materials and/or techniques to perform pulpotomy and pulpectomy, respectively. According to the literature, the existing techniques show good results, with success rates in favor of its use in clinical practice. However the ideal material has not been found. There is no evidence of which material or technique is the most appropriate to perform a pulpotomy or pulpectomy in primary teeth. Further researches are needed in this direction, as well as more studies with higher quality, so that better choices in health care can be made.
