Efeitos pleiotrópicos das estatinas

Abstract

As estatinas, inibidores da 3-hidroxi-3-metilglutaril-coenzima A reductase (HMGR), são uma das actuais terapêuticas para a redução da hipercolesterolémia, sendo esta a classe de fármacos mais prescrita para essa condição. O seu mecanismo de acção é único na inibição da enzima HMGR, enzima limitante da via do mevalonato, que vai originar como produto final o colesterol. A via do mevalonato, porém, origina vários outros produtos para além do colesterol. Esses produtos, enzimas e isoprenóides, estão implicados em várias acções no organismo que poderão estar envolvidas em várias patologias, como as cancerígenas, cardiovasculares, reumatológicas, entre outras. A acção das estatinas na inibição da via do mevalonato vai conferir-lhes então outros efeitos para além do seu propósito principal; esses efeitos, denominados de efeitos pleiotrópicos, poderão constituir um avanço terapêutico para as mais variadas patologias. Efeitos imunomodeladores, efeitos anti-inflamatórios ou anti-proliferativos são exemplos de novos efeitos descritos nas estatinas, que poderão expandir o potencial desta classe de fármacos, que ainda está apenas confinada à redução da hipercolesterolémia.
