Abstract

A endometriose, patologia ginecológica benigna caracterizada pela presença de estroma e glândulas endometriais funcionais em localização extra-uterina, é uma doença estrogénio-dependente e inflamatória, associada a disfunção imunitária. Contudo, a sua etipatogenia não é totalmente conhecida, pelo que a realização deste trabalho se mostrou pertinente. Para tal, foi realizada revisão de literatura selecionada a partir do motor de busca PubMed, privilegiando a pesquisa de artigos de revisão e científicos em língua inglesa e em língua francesa, publicados entre 2011 e 2016.Podem estar na origem da endometriose fenómenos etiopatogénicos tão distintos como a menstruação retrógrada, a disseminação linfática ou vascular, a metaplasia celómica, alterações hormonais, a disfunção do sistema imunitário, a existência de células estaminais endometriais, a vasoconstrição uterina anormal e a predisposição genética. Sabe-se que vários destes mecanismos contribuem para a indução da angiogénese e da proliferação celular, que são essenciais na instalação da lesão endometriótica. Tal como demonstrado ao longo deste trabalho, múltiplas teorias têm sido fundamentadas, o deve levar a acreditar que a etiologia é multifatorial e a fisiopatologia complexa e insuficientemente esclarecida. O conhecimento adquirido ao redigir este trabalho resultou também na proposta de desenvolvimento de uma fórmula preditiva da probabilidade de uma doente ter endometriose. .

Endometriosis, a benign gynecological disease defined as the presence of stroma and functional endometrial glands in extra-uterine placement, is an inflammatory and estrogen-dependent disease related to immune dysfunction. However, its etiopathogenesis is not completely understood and that is why this study is relevant. Researches were made with PubMed search engine, privileging review and scientific articles written in English and French and published between 2011 and 2016.Etiopathogenic phenomena involved can be as distinct as retrograde menstruation, lymphatic or vascular dissemination, coelomic metaplasia, hormonal changes, immune dysfunction, endometrial stem cells, abnormal uterine vasoconstriction and genetic predisposition. It is known that many of these events contribute to induce angiogenesis and cell proliferation, which are necessary to cause endometriotic lesions. As shown in this work, several theories have been supported and that must encourage us to believe that endometriosis is a multifactorial disease and that its physiopathology is complex and insufficiently clarified.The knowledge gathered while writing this thesis culminated with the suggestion of developing a formula that predicts the odds of having endometriosis. . .
