Abstract

Algunos pacientes infectados con el virus de inmunodeficiencia humana (VIH), cuando comienzan la terapia antirretroviral (TAR), presentan un fenómeno paradojal denominado Síndrome Inflamatorio de Reconstitución Inmune (SIRI). Su sistema inmune comienza a reconstituirse, incrementa el recuento de los linfocitos T CD4+ y disminuye la carga viral en plasma, sin embargo, se presenta un deterioro clínico, debido a la aparición de infecciones preexistentes en forma subclínica y a las manifestaciones de enfermedades autoinmunes y/o neoplasias. Por lo tanto, el SIRI se define como una reacción inflamatoria no controlada contra patógenos oportunistas y/o antígenos tumorales que ocurre luego de iniciar la TAR, y estaría asociado con el incremento del recuento de linfocitos CD4+ y con una regulación alterada de la interacción entre los sistemas inmune y endócrino, causando un deterioro clínico de los pacientes. La patogénesis del SIRI es todavía desconocida y los factores de riesgo que conducen a su presentación no están claros. Se cree que el SIRI es el resultado de una respuesta inflamatoria, producto de una interacción inmunoendócrina alterada. En este trabajo se revisará el potencial rol del eje adrenal en el desarrollo del SIRI.
