Pileflebitis esplenoportal: cuando el intestino y el sistema venoso conspiran

Abstract

La pileflebitis, también conocida como trombosis infecciosa del sistema venoso porta, es una complicación grave de infecciones intraabdominales, con una incidencia baja, pero con alta tasa de mortalidad. Reportamos el caso de un hombre de 75 años con antecedentes de hipertensión arterial y cáncer de colon en remisión, quien consultó por diarrea, fiebre, dolor abdominal y coluria. En los estudios paraclínicos se encontró trombocitopenia y elevación de azoados. La tomografía abdominal reveló trombosis de la vena porta y esplénica, gas en la confluencia esplenomesentérica y engrosamiento rectosigmoideo, sin signos de hipertensión portal. Durante la hospitalización, se documentó bacteriemia por Bacteroides fragilis y Escherichia coli productora de beta-lactamasas de espectro extendido (BLEE), confirmándose el diagnóstico de tromboflebitis séptica esplenoportal. El paciente fue tratado con meropenem por seis semanas y anticoagulación por tres meses, con evolución clínica favorable. Este caso resalta la importancia de considerar la pileflebitis en pacientes con infecciones abdominales y signos sistémicos e ilustra la necesidad de un enfoque diagnóstico y terapéutico oportuno y multidisciplinario.
