CRITÉRIOS DE DIAGNÓSTICO E TRIAGEM NO RETINOBLASTOMA

Abstract

O Retinoblastoma (RB) é a neoplasia intraocular primária maligna mais prevalente na infância, decorrente de mutações bialélicas no gene supressor de tumor RB1. A eficácia do tratamento e o prognóstico estão diretamente ligados à precocidade do diagnóstico, sendo a sobrevida superior a 95% em países desenvolvidos, em contraste com regiões onde o atraso diagnóstico é frequente. Clinicamente, os sinais mais prevalentes são a leucocoria e o estrabismo. O diagnóstico é predominantemente clínico, apoiado pelo exame de fundo de olho sob anestesia e por métodos de imagem, com a Ressonância Magnética (RM) sendo o padrão-ouro para avaliar a extensão extraocular. O estadiamento é orientado pela Classificação Internacional de Retinoblastoma Intraocular (ICIR). Inovações incluem a biópsia líquida do humor aquoso para biomarcadores e a triagem genética. O atraso no diagnóstico (lag time), seja parental ou sistêmico, é um fator crítico de pior prognóstico, sublinhando a necessidade de conscientização e capacitação de profissionais de saúde. O tratamento atual visa a preservação ocular com quimioterapia multimodal (intravenosa, intra-arterial e intravítrea), terapias adjuvantes e o reconhecimento do papel da fisioterapia na mitigação dos efeitos neurotóxicos da quimioterapia.
