Nasoalveolar moulding pra pembedahan pada penderita celah bibir dan langit-langit

Abstract

Perawatan bayi dengan celah bibir dan langit-langit (CBL) dilakukan sejak tahun 1950 dengan tujuan untuk melakukan koreksi terhadap cacat tersebut. Nasoalveolar moulding (NAM) hadir untuk mengubah paradigma dari metode konvensional perawatan bayi CBL. Pada metode konvensional terdapat masalah, yaitu kegagalan mengoreksi deformitas kartilago hidung pada penderita CBL unilateral dan bilateral serta defisiensi jaringan kolumela pada bayi dengan celah bibir bilateral. Piranti NAM dibuat dari kawat dan akrilik yang dilekatkan pada feeding plate. Piranti ini dipakai untuk menuntun kartilago hidung, premaksila, dan tulang alveolar pada pertumbuhan dan posisi yang normal pada periode neonatal. Efek penggunaan NAM sebelum dilakukan pembedahan adalah mengurangi tingkat keparahan deformitas oronasal. Teknik NAM memiliki kelebihan mengoreksi kartilago hidung yang immature dan mempertahankan pada posisi yang normal. NAM bertujuan memperpanjang kolumela CBL bilateral sesuai prinsip-prinsip ekspansi jaringan, dengan cara memperpanjang nasal stent dan aplikasi gaya-gaya yang diberikan pada bibir dan hidung. Penggunaan teknik NAM mengurangi surgical scar jika dibandingkan dengan rekonstruksi kolumela konvensional, mengurangi biaya pembedahan dan menjadi standar perawatan pada pusat pelayanan CBL.
