Abstract

El actual sistema de estadificación de la enfermedad renal crónica (ERC) de la Iniciativa para la Calidad de los Resultados de la Enfermedad Renal (KDOQI) es simple pero demasiado rígido para adaptarse a las variaciones en la función renal observadas en la población general. La fórmula más comúnmente utilizada para estimar la función renal no está validada en sujetos sin evidencia a priori de enfermedad renal. Su uso combinado da como resultado un diagnóstico inadecuado de ERC y estimaciones improbables de las tasas de prevalencia. Aunque esta iniciativa ha elevado el perfil de la enfermedad renal, la exageración del alcance del problema podría distraer a los nefrólogos de su papel de especialistas. La comunidad de nefrología necesita un sistema de estadificación revisado para la ERC que permita una comunicación precisa, eficaz y oportuna con los pacientes, los médicos de atención primaria, los médicos de salud pública y los encargados de formular políticas. Su función más importante será identificar a los pacientes que se beneficiarán de la detección selectiva y de intervenciones eficaces y seguras. La siguiente revisión bibliográfica ofrece una definición modificada y un sistema de estadificación de la ERC en función de la presencia de enfermedad renal estructural irreversible e inequívoca, la presencia o grado de deterioro de la función renal y las consecuencias de la misma.
