Abstract

Objectivos: Caracterizar e avaliar o impacto funcional de uma população com persistência ou recorrência de dor após cirurgia vertebral lombar. Material e métodos: A amostra foi constituída por doentes com antecedentes de cirurgia lombar, que mantinham quadro doloroso e que tinham realizado exame imagiológico à coluna lombar após a cirurgia. Foi feita a caracterização demográfica da população, caracterização da dor, utilizando o inquérito DN-4, valorização da clínica neurológica e dos achados em exames de imagem. A avaliação da qualidade de vida foi objectivada com recurso ao inquérito SF-36. Resultados: Foram incluídos no estudo 28 doentes, maioritariamente do sexo feminino e em idade activa. A maioria dos doentes apresentou dor com características neuropáticas, alterações no exame neurológico e em 27 dos 28 doentes foram objectivadas nos exames de imagem compressão de estruturas nervosas endo ou pericanalares, mais frequentemente por recidiva da patologia inicial e fibrose cicatricial. Os resultados obtidos no inquérito SF-36 mostraram claramente o impacto negativo em termos de qualidade de vida destes doentes. Conclusão: Nesta amostra a maioria dos doentes tinha documentação imagiológica de lesões anatómicas passíveis de motivarem o quadro doloroso. O impacto negativo na qualidade de vida dos doentes torna esta condição um verdadeiro desafio terapêutico para a Medicina Física e de Reabilitação.
