Meningite assética associada à terapêutica com imunoglobulina endovenosa

Abstract

A meningite asséptica pode ser uma complicação da terapêutica com imunoglobulina endovenosa. Apresentamos o caso de um rapaz, 4 anos, previamente saudável, internado por Púrpura Trombocitopénica Imune. Recebeu duas doses de imunoglobulina endovenosa. Quatro horas após a última administração iniciou síndrome meníngea. O líquido cefalorraquidiano revelou 500células/μL (predomínio neutrófilos), com bioquímica normal. Os exames bacteriológico e virulógico foram negativos. Ficou assintomático após 48h. Dada a ausência de outro factor etiológico e a relação temporal com a administração da imunoglobulina, admitimos tratar-se de uma meningite pós imunoglobulina endovenosa. Esta terapêutica pode causar cefaleias, febre e vómitos; contudo, habitualmente não se efectua punção lombar, estando esta complicação eventualmente subdiagnosticada.
