Mastocitose Cutânea Difusa — Um Caso Clínico

Abstract

A mastocitose é uma entidade clínica heterogénea caracterizada pela infiltração dos diferentes tecidos e orgãos por mastócitos. A mastocitose cutânea difusa é uma forma rara que ocorre quase exclusivamente no lactente, com complicações potencialmente fatais e maior probabilidade de envolvimento sistémico e evolução para a cronicidade. Os autores descrevem o caso de um lactente com mastocitose cutânea difusa na forma bolhosa, com manifestações sistémicas, lesões cutâneas urticariformes e angioedema. A pele nunca teve o aspecto classicamente descrito e o diagnóstico foi apenas efectuado aos 14 meses aquando de um surto vesicobolhoso grave, que motivou internamento. O tratamento incluiu evicção dos factores causadores de desgranulação mastocitária e/ou libertadores de histamina, hidroxizina e ketotifeno sendo a evolução ulterior favorável. A propósito deste caso os autores discutem a etiopatogenia, manifestações clínicas que devem_ alertar para o diagnóstico, bem como a terapêutica e o prognóstico.
