Cáncer colorrectal sincrónico con resección multisegmentaria: desafíos diagnósticos y terapéuticos. Reporte de caso

Abstract

El cáncer colorrectal múltiple es una afección rara, con controversias actuales en su diagnóstico y manejo dados los pocos reportes en la literatura. Se presenta el caso de un paciente de 58 años, sin antecedentes de relevancia, que acude al servicio de urgencias por un cuadro de hematoquecia y pérdida de peso. La colonoscopia mostró lesiones de aspecto neoplásico en el ciego y el colon sigmoide, con confirmación histopatológica de adenocarcinoma bien diferenciado e infiltrante. Se realizó una hemicolectomía derecha con anastomosis ileotransversa latero-lateral intracorpórea isoperistáltica y resección anterior alta de recto con anastomosis colorrectal termino-terminal con grapadora circular, mediante laparoscopia. La patología reportó adenocarcinoma moderadamente diferenciado con márgenes negativos, invasión linfovascular presente y baja probabilidad de inestabilidad de microsatélites. En casos de cáncer colorrectal múltiple, la resección quirúrgica multisegmentaria puede ser un procedimiento factible en pacientes seleccionados, con la ventaja de preservar una mayor funcionalidad posquirúrgica al conservar colon sano. En el enfoque terapéutico de estos pacientes se deben tener en cuenta los factores de riesgo, las herramientas diagnósticas y las condiciones clínicas para la elección del procedimiento quirúrgico adecuado y mejorar el pronóstico de la enfermedad.
