A communications form should furnish the patient information required under Medicare and convey the many specific facts which the nurse needs from the moment a patient becomes her responsibility in order to understand him and to plan his care. The form described here was designed to do this and was tested and refined over a three-year period by nurses, physicians, social workers, and others concerned with continuity of care.
