Lipodistrofia em doente com cirrose hepática alcoólica

Abstract

A lipodistrofia, também denominada doença de Madelung é uma doença rara, caracterizada pelo depósito de gordura não encapsulada, que se distribui de forma simétrica ao redor do pescoço e ombros. A sua etiologia é ainda desconhecida, sendo frequentemente observada em doentes com história de alcoolismo crónico. As principais manifestações são anormalidades estéticas e metabólicas. O tratamento é a remoção cirúrgica do tecido adiposo - lipectomia ou lipoaspiração- com recorrência comum em alguns casos. Apresenta-se o caso de um doente do sexo masculino, de 59 anos, com antecedentes pessoais de alcoolismo crónico internado no serviço de Medicina Interna por descompensação de cirrose hepática alcoólica (Classe C de Child-Pugh). Ao exame objectivo, observavam-se depósitos volumosos de gordura na região anterior e posterior do pescoço, no tronco, na região escapular e em ambos os braços até aos cotovelos, configurando o aspecto característico desta patologia. Será mostrada documentação fotográfica do caso.
