Abstract

片頭痛の病態は長い研究の歴史をもつが,いまだに明らかになっていないのが現状である.それは,血管説・神経説・三叉神経血管説など病態各説がニューロトランスミッターを介して相互に関わっているためと,病変の主座である脳血管の病的変化の鍵を握る分布する神経線維のニューロトランスミッター自体の種類・作用が十分解明されていないためである.本稿では,このニューロトランスミッターに焦点をあて,最近の知見を中心に現在の片頭痛の病態研究の話題と問題点を浮き彫りにすることを目的とした.片頭痛に関わるニューロトランスミッターには,血管反応性に関与するものと痛覚伝達に関与するもの,およびに2つの機能を兼ね備えるものに分けられる.治療への応用という見地からもっとも注目されているものはセロトニン(5-HT)で,とくに5-HT〓受容体の刺激は血管収縮作用とともに血管周囲に生じるneurogenic inflammationをも抑制することにより片頭痛発作を抑制しうる.また, substance Pをはじめとするtachykinin, calcitonin gene-related peptideあるいは一酸化窒素(NO)などは痛覚伝達とneurogenic inflammationの発生の双方に関与している.また,最近抑制性アミノ酸GABAの片頭痛発作抑制作用が注目されている.片頭痛の病態研究は近年ニューロトランスミッターからのアプローチにより飛躍的に進展したが,いまだ全貌は明らかになっておらず今後の発展が期待される.
