Abstract

Gli investimenti sanitari e formativi che gli individui effettuano per mantenere ad elevato livello il proprio stock di salute e di conoscenze costituiscono i fondamenti del capitale umano e sociale che caratterizza la società in cui essi vivono. La società ha ragioni morali ed economiche per adottare politiche che incentivino gli investimenti autointeressati e creino condizioni di eguaglianza fra gli individui nel realizzarli. La nostra attenzione è qui concentrata sui problemi di eguaglianza e diseguaglianza nella formazione del capitale sanitario e sul ruolo che le politiche sanitarie pubbliche possono assumere. I dibattiti mostrano che, a dispetto della loro asserita meritorietà, queste devono spesso superare gravi ostacoli di natura ideologica, oltre che di sostenibilità finanziaria. Esaminiamo, al riguardo, alcuni modelli di filosofia politica che negli Usa limitano gli obiettivi di eguaglianza e di estensione della riforma Obama. E ci rivolgiamo infine al retroterra culturale e sociale che in Italia ostacola l’eguaglianza sanitaria territoriale fra Nord e Sud, che è stata un obiettivo qualificante della riforma n. 833/1978.
