Colonoscopia hoy: ¿real o virtual?

Abstract

Permitanme que me sincere: pocas veces ha habido tanta imprecision en el titulo de un manuscrito. Y con ello no me refiero a la interrogacion. La vaguedad es intrinseca a los dos adjetivos empleados para cualificar la colonoscopia. ?Que es la colonoscopia «real»? ?A que nos referimos cuando hablamos de colonoscopia «virtual»? El diccionario de la RAE define real como «que tiene existencia verdadera y efectiva». En base a esta definicion, es evidente que tan «real» es la colonoscopia optica (denominada erroneamente, en muchas ocasiones, «convencional», puesto que este concepto es aun mas temporal y ambiguo) como la colonografia-TC. No obstante, para muchos gastroenterologos, dificilmente puede existir una exploracion mas «real» que la primera. Asi, durante muchos anos, ha habido la creencia, mas o menos interesada, de que la colonoscopia era una exploracion infalible. El hecho de que esta se haya empleado como patron oro en la evaluacion de nuevas tecnicas diagnosticas ha contribuido a ello1. Sin embargo, existe cada vez mayor evidencia de que esta exploracion presenta limitaciones en el diagnostico de algunas lesiones, en especial aquellas de menor tamano, planas y/o situadas en el colon proximal. La demostracion de esta circunstancia procede, fundamentalmente, de estudios endoscopicos en los que se han realizado dos exploraciones consecutivas por distintos examinadores (back to back)1, pero en otras ocasiones han sido los ensayos dirigidos a evaluar el ren-
