Abstract

RESUMEN: La Seguridad del Paciente (SP), o el intento consciente de evitar lesiones al paciente causadas por la asistencia, es un componente esencial de la Calidad Asistencial y la condición previa para la realización de cualquier actividad clínica. Solo recientemente a la luz de datos objetivos, la seguridad del paciente ha comenzado a considerarse una cuestión primordial en la práctica sanitaria.Los fundamentos de la seguridad asistencial se asientan en dos líneas de pensamiento relacionadas: 1. La teoría del error de Reason y 2. La cultura de seguridad (CS).La cultura de seguridad, se define como el conjunto de valores y normas comunes a los individuos dentro de una misma organización e implica un modelo mental compartido que posiciona la seguridad como un objetivo común a perseguir.Este artículo explora las definiciones, procesos y estrategias encaminados a reducir la probabilidad de aparición de fallos del sistema y errores de las personas y aumentar la probabilidad de detectarlos cuando ocurren y mitigar sus consecuencias. SUMMARY: Patient Safety (PS), or the conscious attempt to avoid patient injury caused by attendance. Is an essential component of the quality of care and the precondition for the performance of any clinical activity.Only recently in the light of objective data, patient safety has begun to be considered a paramount issue in health practice.The fundamentals of health care security are based on two related lines of thought:1. The Reason Error Theory, and 2. The safety culture (SC).The safety culture is defined as the set of values and norms common to individuals within the same organization and implies a shared mental model that positions security as a common goal to pursue.This article explores the definitions, processes and strategies aimed at reducing the probability of occurrence of system failures and errors of people and increase the probability of detecting them when they occur and mitigating their consequences.
